Payment information, which is processed by our third-party payment processor. We do not store your full payment card number.
Website usage information collected through cookies and similar technologies, used to operate and improve our website.
How We Use and Disclose Your Information
We use and disclose PHI for treatment (for example, sharing your intake with the licensed healthcare provider reviewing your case and with the licensed pharmacy filling your prescription), for payment, and for healthcare operations. We share information with service providers — such as our telehealth platform, records vendors, and partner pharmacies — only under agreements that require them to protect it. We may disclose information when required by law, to prevent a serious threat to health or safety, or in connection with a corporate transaction, subject to applicable law.
We do not sell your personal or health information, and we do not use your health information for third-party advertising.
Your Rights
You have the right to: access and obtain a copy of your health records; request corrections to your records; request restrictions on certain uses and disclosures; request that we communicate with you in a certain way or at a certain location; receive an accounting of certain disclosures; receive notice of any breach affecting your unsecured PHI; and receive a paper copy of this notice on request. You will not be penalized or retaliated against for exercising any of these rights or for filing a complaint.
