Consent to Telehealth Services

By using Improve Wellness's services, you consent to receive healthcare services via telehealth. Telehealth means the delivery of healthcare services using electronic information and communication technologies between a patient and a healthcare provider who are not in the same physical location.

Expected Benefits

Telehealth provides convenient access to licensed healthcare providers without the need for in-person visits, including medical evaluation, treatment recommendations, and prescriptions where medically appropriate.

Possible Limitations

Your provider will not perform a hands-on physical examination and will rely on the information, and any photographs, you provide. You agree to provide complete and accurate information. Telehealth is not appropriate for every condition; your provider may determine that you require in-person care and refer you accordingly.

Possible Risks

As with any healthcare service, there are potential risks, including delays in evaluation or treatment due to technical failures, and, despite security safeguards, risks inherent in the electronic transmission of information.

Your Rights

You may withhold or withdraw your consent to telehealth at any time without affecting your right to future care or treatment. If you withdraw consent, telehealth services may not be able to be provided to you. Your medical information will be protected as described in our Privacy Policy, and the same confidentiality protections that apply to in-person care apply to telehealth

Florida Patients

Telehealth services provided to Florida patients are provided in accordance with Section 456.47, Florida Statutes, by providers authorized to provide telehealth services in Florida

Telehealth services are not intended for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room

U.S. Telehealth Availability & State Regulations

Our telehealth services are available to patients located in all 50 U.S. states and the District of Columbia. All consultations are conducted by licensed U.S. healthcare providers, and all prescriptions are dispensed by licensed U.S. pharmacies. Treatment is only provided where clinically appropriate and permitted by applicable state and federal law. Availability of specific medications may vary by state based on state regulations and provider licensure. Asynchronous (online questionnaire) care is available in the following 44 states: AK, AL, AR, AZ, CA, CO, CT, DE, FL, GA, HI, IA, ID, IL, IN, KY, MA, MD, ME, MI, MN, MO, MT, NC, ND, NE, NH, NJ, NV, NY, OH, OK, OR, PA, SC, SD, TN, TX, UT, VA, VT, WA, WI, WY. A live (synchronous) consultation with a provider is required before treatment in these 7 jurisdictions: District of Columbia (DC), Kansas (KS), Louisiana (LA), Mississippi (MS), New Mexico (NM), Rhode Island (RI), West Virginia (WV).

Medical Group & Pharmacy Network

We are partnered with DrTelx (drtelx.com), an independent telehealth medical group whose providers are licensed in the states where we offer care. We work exclusively with licensed U.S. pharmacies that meet rigorous regulatory and quality standards. Our pharmacy partners include EPIQ Scripts, Foothills Pharmacy, and Emerald Compounding Pharmacy. Every pharmacy in our network is licensed in the states where it dispenses, accredited by the National Association of Boards of Pharmacy (NABP), board-certified, and LegitScript-approved. All prescriptions are filled and shipped by these licensed U.S. pharmacies in compliance with applicable state and federal law.
